Job Summary
RN Case Manager (Contract) for Vitra Health in Massachusetts. Conduct in-person intake, nursing assessments, and care planning; coordinate and monitor care; educate clients and caregivers on health maintenance, disease prevention, and available community resources; participate in discharge planning and multidisciplinary team meetings; complete documentation and ensure compliance with regulatory requirements. Requires a current MA RN license, 2+ years direct elder/adult-with-disability care, valid driver’s license, reliable transportation, ability to travel regionally, and strong communication and organizational skills. Travel throughout assigned regions; in-home visits; office-based documentation; tablet-based recordkeeping. Travel-heavy field role within AFC/GAFC and community health settings.
Required Qualifications
- Current Massachusetts nursing license in good standing (RN)
- Minimum of two (2) years of recent experience providing direct care to elders or adults with disabilities
- Valid driver's license, reliable transportation, and proof of insurance
- Willingness and ability to travel throughout assigned regions
- Strong communication, interpersonal, documentation, and organizational skills
- Ability to work independently while effectively managing multiple priorities
- Strong problem-solving and critical-thinking abilities
Desired Qualifications
