Job Summary
Conduct comprehensive assessments of patients' medical, social, psychological, and financial needs to identify discharge barriers and develop individualized care transition plans. Coordinate healthcare services by collaborating with physicians, nurses, social workers, and insurance providers to ensure continuity of care. Manage caseloads by monitoring progress, evaluating interventions, and adjusting care plans to optimize outcomes and resource utilization. Advocate for patients and families, ensuring their rights and preferences are respected while educating them on treatment options and discharge instructions. Maintain accurate documentation of all activities to ensure compliance with policies and regulatory requirements. This Full-Time, 1.0 FTE role includes rotating weekends and holidays, with no on-call expectations. We offer a $5,000 sign-on bonus and relocation assistance to qualified candidates.
Required Qualifications
- Nursing Degree from an approved school of nursing
- Active Registered Nurse license in good standing with the North Dakota Board of Nursing or Active Registered Nurse license in a compact state
- Working knowledge of patient care across various healthcare settings, care coordination principles, and utilization review
- Basic Life Support (BLS) Certification within the orientation period and meets re-certification as required
- Good communication skills necessary for constant communication, both oral and written, with patients, families, visitors, and medical center staff
- Accountable for own conduct and promotes and supports positive working relationships among staff, between other disciplines, departments, families, visitors, and patients
- Maintains confidentiality
- Active Registered Nurse license in good standing with the North Dakota Board of Nursing or Active Registered Nurse license in a compact state
