Coordinate clinical care for a designated patient population across the continuum of care, utilizing advanced nursing skills and knowledge of resource management and fiscal responsibility. Facilitate quality-based, cost-effective patient outcomes by interacting with Medical, Nursing, and Ancillary department staff to ensure care is provided in the most appropriate setting. Manage clinical effectiveness, discharge planning, and clinical resource management to support the region's adult academic health system in advancing healing and reducing suffering for community and international patients.
