Job Summary
Per Diem RN Case Manager coordinating and delivering palliative and supportive care through the Interdisciplinary Group (IDG); assesses patient/family needs, develops and initiates Plan of Care, educates patients/families, and ensures timely admissions and communication with physicians and facilities. Requires an active RN license, BLS, 1+ year nursing experience (hospice/hospital preferred), EMR/EHR experience, valid driver’s license, reliable transportation, English proficiency, and ability to perform essential clinical and physical tasks (lifting, mobility, travel) with/without accommodation; some locations may require Alzheimer’s training and cross-training for flexible staffing. Compensation: $47.00 per hour. Location: Falls Church, VA.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Minimum one (1) year nursing experience; hospice or hospital experience preferred
- Experience working with an EMR/EHR system
- Valid driver’s license and automobile insurance per Company policy
- Reliable transportation to meet visit schedule
- Ability to use equipment with visual and auditory mechanisms
- Ability to effectively communicate in English (verbal and written)
- Flexible schedule to meet the needs of Hospice House patients
- Ability to manage emotional stress of caring for terminally ill patients and families
- Physical ability to access patient rooms and navigate stairs and narrow spaces
- Ability to perform essential functions and physical requirements with/without accommodation
- Completion of Alzheimer's Disease and Related Dementias Training may be required at some locations
- Cross training to hospice house or field positions may be required
- Completes clinical admissions for all Company services
- Explains services to potential patients/families and addresses questions
- Educates patient/family on admission requirements
- Obtains Physician orders and initiates Plan of Care
- Maintains documentation per Company policy
- Works with Admissions and IDG; SBAR communications
- Supervises nursing team members
- Travel and potential after-hours/overnight work during emergencies
