RN Case Manager

RN Case Manager coordinates services and develops individualized care plans for patients, guiding families through the care journey. Responsibilities include coordinating education for patients, families, and caregivers; completing psychosocial assessments and discharge planning evaluations; collaborating with the Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care; explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights; promoting safe transitions and clear communication across the care continuum; and upholding HCA Healthcare’s values of compassion, respect, and excellence. Typical schedule is Monday–Friday, 8:00 AM–4:30 PM in a hospital setting.
