RN Case Manager, Care Management, Part Time, First Shift

Coordinate clinical care for a designated patient population across the continuum of care, utilizing advanced nursing skills and knowledge of resource management and fiscal responsibility. Facilitate quality-based, cost-effective patient outcomes by interacting with Medical, Nursing, and Ancillary department staff to ensure care is provided in the most appropriate setting. Manage discharge planning, care coordination, and clinical resource management to support clinical effectiveness. This role is part-time, first shift, located at UC Health, an academic health system dedicated to advancing healing and reducing suffering.
