Job Summary
Respond to clinical referral information with care, assessing client eligibility for specialized hospice services. Partner with the Clinical Director to guide care planning, ensuring patients and families receive comfort and quality of life. Deliver hands-on nursing care rooted in clinical excellence while educating families to create personalized care plans. Share responsibility for call duties to support the interdisciplinary team during critical moments. This Full Time role covers Dover, Durham, Rochester, Somersworth, and Portsmouth, Monday through Friday, 8am to 4:30pm. Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license, vehicle insurance, and access to dependable transportation
