Job Summary
RN Case Manager role delivering compassionate, one-on-one hospice care and coordinating individualized care plans for patients and families. Responsibilities include conducting comprehensive physical, emotional, and spiritual assessments; monitoring vital signs; administering physician-ordered medications; educating patients and families on care techniques and symptom management; maintaining up-to-date clinical notes; collaborating with interdisciplinary teams; and taking on additional duties as part of a mission-driven team. Requires an active RN license in the working state, CPR certification, a valid driver’s license with reliable transportation, strong observation and written/verbal communication skills, and a flexible, team-oriented approach.
Required Qualifications
- A valid Registered Nurse license in good standing in the state you will work in
- CPR certification (current)
- A valid driver’s license and reliable transportation
- Strong observational and communication skills—both written and verbal
- Flexibility and team-oriented attitude; willingness to support additional duties as needed
