Job Summary
RN Case Manager delivering compassionate hospice care by developing and implementing individualized care plans, conducting comprehensive physical, emotional, and spiritual assessments, monitoring vital signs, administering physician-ordered medications, educating patients and families on care techniques and symptom management, documenting detailed clinical notes, and collaborating with interdisciplinary teams to ensure holistic care. Requires valid RN license, CPR certification, and a valid driver’s license with reliable transportation. Travel between Mission, TX and Bristol Hospice McAllen, TX locations may be involved; mileage reimbursement is provided. The role emphasizes advocacy, patient comfort, and ensuring dignity throughout the hospice journey.
Required Qualifications
- A valid Registered Nurse license in good standing in the state you will work in
- A valid driver’s license and reliable transportation
- CPR certification (current)
- Strong observational and communication skills—both written and verbal
- A flexible, team-oriented attitude and willingness to support additional duties as needed
