RN Case Manager

RN Case Manager responsibilities include developing and implementing individualized care plans based on patient medical history, delivering one-on-one support to patients and families to ensure comfort and dignity, conducting comprehensive assessments of physical, emotional, and spiritual needs, monitoring vital signs, administering physician-ordered medications, educating patients and families on care techniques and symptom management, tracking changes in patient condition with timely communication, maintaining up-to-date clinical notes, collaborating with interdisciplinary teams to ensure comprehensive care delivery, and contributing to other supportive duties as part of a compassionate, mission-driven team.
