RN Case Manager

RN Case Manager delivering patient- and family-centered hospice nursing care in Wisconsin. Responsibilities include developing and implementing individualized care plans based on medical history, delivering one-on-one support to patients and families, conducting comprehensive physical, emotional, and spiritual assessments, monitoring vital signs, administering physician-ordered medications and treatments, educating families on care techniques and symptom management, tracking changes in patient condition with timely communication, maintaining up-to-date clinical notes, and collaborating with interdisciplinary teams to ensure comprehensive care. Requires a valid RN license, CPR certification, and a valid driver’s license with reliable transportation; strong observational and communication skills; flexibility to support additional duties as needed.
