Job Summary
As an RN Case Manager at Bristol Hospice, you will develop and implement individualized care plans, deliver one-on-one support to patients and their families, conduct thorough assessments of patient needs, administer medications and treatments, educate families on care techniques, and track patient conditions. You will collaborate with interdisciplinary teams to ensure comprehensive care delivery. Key qualifications include a valid RN license, a valid driver’s license, and current CPR certification.
Required Qualifications
- Valid Registered Nurse license in good standing
- Valid driver's license and reliable transportation
- Current CPR certification

- Strong observational and communication skills—both written and verbal
- A flexible, team-oriented attitude and willingness to support additional duties as needed
