Job Summary
Provide individualized, compassionate hospice nursing care as an RN Case Manager; develop and implement care plans based on medical history, monitor vital signs, administer physician-ordered medications, educate patients and families on care techniques, document changes in patient condition, and collaborate with interdisciplinary teams to ensure comprehensive, high-quality end-of-life care. The role emphasizes advocacy, family support, thorough assessments (physical, emotional, and spiritual), and ensuring comfort, dignity, and understanding throughout the care journey.
Required Qualifications
- A valid Registered Nurse license in good standing in the state you will work in
- CPR certification (current)
- A valid driver’s license and reliable transportation
- Strong observational and communication skills—both written and verbal
- A flexible, team-oriented attitude and willingness to support additional duties as needed
