RN Case Manager

Develop and implement individualized care plans based on current and prior medical history, delivering attentive, one-on-one support to patients and families to ensure comfort, dignity, and understanding. Conduct thorough assessments of physical, emotional, and spiritual needs while accurately monitoring vital signs and administering physician-ordered medications with skill and empathy. Educate patients and families on care techniques and symptom management, then track and communicate changes in patient condition to ensure timely responses. Maintain detailed clinical notes reflecting patient progress and collaborate with interdisciplinary teams to ensure comprehensive care delivery.
