Job Summary
Develop and implement individualized care plans based on current and prior medical history for patients in Lorain and Medina counties. Deliver attentive, one-on-one support to patients and families, ensuring comfort, dignity, and understanding while conducting thorough assessments of physical, emotional, and spiritual needs. Accurately monitor and document vital signs, administer physician-ordered medications and treatments, and educate families on care techniques and symptom management. Track and communicate changes in patient condition, keeping detailed clinical notes to reflect patient progress. Collaborate with interdisciplinary teams to ensure comprehensive care delivery and contribute to other supportive duties as assigned. Work Monday through Friday from 8am to 5pm with some holiday and PTO coverage.
Required Qualifications
- A valid Registered Nurse license in good standing in the state you will work in
- A valid driver's license and reliable transportation
- CPR certification (current)
- Strong observational and communication skills—both written and verbal
