RN Case Manager

Develop and implement individualized care plans based on current and prior medical history while conducting thorough assessments of physical, emotional, and spiritual needs. Deliver attentive, one-on-one support to patients and families, ensuring comfort, dignity, and understanding through accurate monitoring of vital signs and administration of physician-ordered medications. Educate patients and families on care techniques and symptom management, then track and communicate changes in patient condition to ensure timely responses. Maintain detailed clinical notes reflecting patient progress and collaborate with interdisciplinary teams to ensure comprehensive care delivery as part of a compassionate, mission-driven hospice team.
