Job Summary
RN Case Manager at Bristol Hospice develops and implements individualized care plans, delivers one-on-one support to patients and families, conducts comprehensive assessments of physical, emotional, and spiritual needs, monitors vital signs, administers physician-ordered medications, educates patients and families on care techniques and symptom management, tracks patient condition changes, maintains up-to-date clinical notes, and collaborates with interdisciplinary teams to ensure compassionate end-of-life care.
Required Qualifications
- Valid Registered Nurse license in good standing in the state you will work in
- Valid driver’s license and reliable transportation
- CPR certification
