RN Case Manager

Facilitate patient care plans and lead daily huddles to direct timely progress along the continuum of care. Manage input from all health care professionals to determine transitional needs, ensuring appropriate admissions and utilization of resources. Collaborate with physicians, community case managers, and payers to develop safe discharge plans and overturn denials for appropriate reimbursement. Conduct extensive assessments for patients at risk of readmission and direct care to other settings when appropriate. Requires a BSN, 3-5 years clinical nursing experience, and Case Management certification. Weekend and holiday commitment required. Base salary range $98,051 to $133,348.
