Job Summary
Assess physical and emotional needs of patients and families to coordinate care plans that maximize comfort and quality of life. Document findings in the EMR, update interdisciplinary treatment plans, and communicate status changes to physicians. Administer medications, evaluate nursing interventions, and provide direction to HHA/CNA staff while reporting infections and attending to post-death arrangements. Conduct community visits in Collier County under physician orders, with availability for holiday, on-call, weekend, and illness coverage.
Required Qualifications
- Bachelor's degree (B.A.) from four year college or university
- Current RN License to practice in the state of Florida
- Valid Florida Driver's License
- Healthcare Provider CPR
- Communication skills of reading, writing, and speaking in English
- Math skills including addition, subtraction, multiplication, division, and percentages/fractions
- Must successfully pass the math test given at time of interview prior to starting orientation
- Must be available for holiday, on-call, weekend, illness and vacation coverage as needed
- Must be able to lift and/or move up to 50 pounds
- Must be able to see, hear, and talk
- Must be able to sit, stand, walk, and reach with hands and arms
- Must be able to use hands to finger, handle, or feel
- Must be able to stoop, kneel, crouch, or crawl
- Must be able to be exposed to outside weather conditions
- Must be a Nicotine Free individual
Desired Qualifications
- One to two years related experience and/or training
- Spanish language skills
- Strong computer skills including database software and general knowledge of e-mail/Outlook
