RN Case Manager

Primary functions include administering skilled nursing care for clients of all ages in their place of residence, coordinating care with the interdisciplinary team, patients and their families, and a referring agency. Responsibilities include completing initial and ongoing assessments of home health patients, developing and revising plans of care, referring to other services as needed, maintaining clinical documentation, and supervising nursing personnel. The role requires reliable transportation, ability to work flexible hours, and the capacity to perform under stress while lifting up to 50 lbs.
