Job Summary
Case manage and provide clinical direction to the interdisciplinary team, physician, and family. Implement and document the patient plan of care while utilizing infection control measures to protect staff and patients. Monitor assigned cases to ensure compliance with third-party payer requirements and supervise hospice aides and licensed practical nurses. Provide on-call service to patients and families as assigned. Perform other nursing duties as assigned by management to support department goals. Affinity Hospice is a privately held organization dedicated to exceptional hospice and palliative care, offering physical, emotional, psychosocial, and spiritual support to patients and their families within the communities they serve.
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State license as a Registered Nurse
- Current CPR certification
- Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Perform on-call responsibilities and provide on-call service to patients/families as assigned
Desired Qualifications
- Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred
