Job Summary
Conduct comprehensive physical, social, and psychological assessments for home health and hospice patients. Collaborate with patients, families, and the Interdisciplinary Group to create individualized care plans. Manage care delivery and maintain meticulous patient records while acting as a resource for families through proactive referrals. Work full-time Monday through Friday from 8am to 5pm with one 24-hour oncall shift per month. Earn $40-$45 per hour in Scottsdale, AZ.
Required Qualifications
- Active Registered Nurse (RN) license in Arizona (or state of practice)
- Valid driver's license
- Reliable transportation
- Proof of current auto insurance
- Ability to perform patient transfers with a full range of movement
- Current CPR certification
- Negative TB test
- Valid AZ Fingerprint Clearance Card
- Must be able to pass a criminal background check
Desired Qualifications
- Previous Home Health or Hospice experience
