RN Case Manager Acute Rehab

RN Case Manager Acute Rehab coordinates care for rehabilitation patients by blending clinical nursing expertise with care coordination and discharge planning. Works with an interdisciplinary team to assess clinical status, functional and psychosocial needs, develop comprehensive care plans aligned with rehab goals, educate patients and families on treatment plans and community resources, and coordinate post-acute services (e.g., home health, outpatient therapy, DME). Also communicates with payers and external providers to support authorization and utilization management, ensures safe transitions, monitors progress, and maintains regulatory compliance within hospital policies. Strong clinical judgment and effective communication in a fast-paced, team-based environment are required.
