RN Case Manager - $5,000 Sign On Bonus

Conduct comprehensive assessments of patients' physical, psychosocial, and environmental needs, including ADLs and grief risks. Implement individualized plans of care, recommend revisions, and collaborate with physicians, caregivers, and interdisciplinary groups to coordinate patient care. Perform on-call responsibilities and submit all documentation timely. Ensure compliance with local, state, federal laws, Medicare regulations, and personnel policies. Requires current RN license, two years of clinical experience, and a driver's license with valid auto insurance.
