Job Summary
Deliver skilled nursing care to patients in their homes and community facilities while completing comprehensive assessments of physical, emotional, and spiritual needs. Develop and update individualized care plans, educate families on symptom management and resources, and coordinate interdisciplinary communication to ensure continuity of services. Maintain accurate documentation and advocate for patient goals throughout the hospice journey. This full-time day shift role offers flexible scheduling, no night on-call, and one-on-one time with patients in their home environment.
Required Qualifications
- Current Registered Nurse (RN) license in the State of Maine
- Current CPR certification
- Reliable transportation with valid driver's license and current automobile insurance
Desired Qualifications
- Minimum of one year of recent clinical nursing experience preferred
- Experience in hospice, home health, palliative care, or community health is preferred
- ANA certification in Home Health or Community Health Nursing is encouraged
- Excellent assessment, communication, and critical thinking skills
