RN Case Manager (3649)

Provide skilled nursing services to patients in their own homes under physician direction. Develop plans of care with realistic outcomes in collaboration with patients and families. Assess holistic health care needs and demonstrate competency in assessment and technical skills. Promote patient rights and responsibilities for informed decision-making while managing pain control and symptom management. Utilize knowledge of home health standards to work autonomously in community care settings. Requires current Maine RN license, ANA certification in home/community health nursing, and one year of recent clinical experience. Full-time 40-hour schedule with flexible options available.
