Job Summary
TraumaRN Care Manager coordinates multi-disciplinary resources to facilitate patient transitions across the care continuum. Develops and implements a plan of care to achieve high-quality outcomes in a cost-effective manner, integrates committee decisions into department operations, facilitates access to care transition services, PCPs and specialists, and communicates with patients, families, and healthcare teams. Responsibilities include assessing patient needs, collaborating with the care team and family to develop a care plan, determining appropriate level of care, coordinating interdisciplinary services, monitoring progress, addressing barriers to progression, and advocating for patient access to services and insurance benefits while balancing cost and quality. The role requires RN licensure in Wisconsin, BLS, and CCM/ ACM certification within one year if ADN-educated. The position is based in Appleton, WI, in a climate-controlled facility with regular movement within the campus; it is a part-time (0.1 FTE) day-shift role within ThedaCare’s trauma care management continuum.
Required Qualifications
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required
- ADN candidates must also hold a bachelor’s degree in a related field (e.g., Psychology, Social Work, Health Science, or equivalent)
- Current Wisconsin RN License
- American Heart Association Healthcare Provider Basic Life Support (BLS)
- National Certification in Care Management (e.g., CCM or ACM) required within one year of hire for candidates with an ADN
