Job Summary
RN Care/Case Manager coordinates care for Community Care members, conducts phone and in-person assessments to identify health needs, completes documentation and charting, and supports development of member care plans. Requires an Associate Degree in Nursing and a current Wisconsin RN license with at least 2 years of relevant experience; full-time daytime schedule (8:00 a.m.–4:30 p.m.) with hybrid work options from Walworth/Wisconsin-based offices. Compensation package includes benefits and an $8,000 sign-on bonus. Based in Rock/Jefferson County, serving Wisconsin residents, with a focus on home care needs and transportation coordination.
Required Qualifications
- Education: Associate degree in nursing required
- Experience: Minimum 2 years of personal, clinical leadership, and/or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred
- Licensure: A current Wisconsin Registered Nurse license in good standing
