Job Summary
RN Care/Case Manager responsibilities include collaborating with the site management team to standardize care delivery for Community Care members, addressing member appointments, transportation, and home care needs; conducting phone and in-person assessments to identify health needs; completing documentation and maintaining member records; position based in Rock County with a Walworth office and hybrid work option; schedule 8:00 a.m.–4:30 p.m., 40 hours per week. Requires an Associate degree in nursing, Wisconsin RN license, and 2 years of relevant care or case management experience with an elderly/frail population; prior clinical leadership experience preferred. The role emphasizes care coordination, documentation, care planning, and collaboration to support independent living for Wisconsin residents.
Required Qualifications
- Education: Associate degree in nursing required.
- Experience: Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
- Licensure: A current Wisconsin Registered Nurse license in good standing
