RN - Care Manager

Assess and plan care for at-risk patients in an acute care environment while coordinating resources, managing discharge planning, and documenting interventions. Ensure regulatory compliance and advocate for patients throughout the continuum to optimize healthcare resource utilization. Requires an Associate's Degree in Nursing, Washington Registered Nurse License, and two years of acute care experience. One year of care management or utilization review experience is necessary, or successful completion of the TIPS program or Case Management Orientation Program. This role supports Providence St. Peter Hospital's mission of providing whole-person care to vulnerable community members.
