Job Summary
RN Care Manager in the PACE program at Piedmont Health Senior Care in Burlington, NC. Responsibilities include providing direct and indirect nursing care to assigned patients, utilizing critical thinking to assist in managing chronic diseases, applying advanced nursing knowledge within NCBON scope, collaborating with the interdisciplinary team (IDT), participating in call rotation, performing home visits, and delivering end-of-life/palliative nursing care per nursing protocol. Requires RN licensure in NC, graduation from an accredited school of nursing, and strong interpersonal, oral, and written communication skills; preferred experience includes home care or long-term care and CMS-approved competencies.
