Job Summary
Facilitate transition of care planning with an interdisciplinary healthcare team to achieve quality outcomes, reduce costs, and ensure patient safety. Coordinate care management activities for patients requiring acute care hospital setting experience, utilizing strong assessment, critical thinking, and communication skills. Manage case management responsibilities while adhering to InterQual criteria where applicable. Requires current nursing licensure, BLS certification at hire or within 90 days, and ability to lift 50 pounds regularly with prolonged standing. Part-time weekend schedule available.
Required Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing
- Current License in the state of employment
- 2-3 years acute care hospital setting
- BLS (CPR) at hire date, Minimum Required, or within 90 days of hire
- Position requires individuals to push, pull, and/or lift 50 pounds regularly
- Position requires prolonged standing and walking during each shift
- Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties
- Strong verbal and written communication and facilitation skills, Minimum Required

Desired Qualifications
- Care Management or Utilization Management experience
