Job Summary
Assess, plan, advocate, and coordinate care from admission to discharge, ensuring a safe transition post-hospitalization. Perform duties consistent with the Mercy mission, values, and service standards while utilizing performance metrics and best practices to oversee transition planning and length of stay. Maintain patient experience, safety, and quality of care within the interdisciplinary team. Requires current RN license, BLS certification, and 2-3 years acute care experience. Part-time days schedule available in Lebanon.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current License in the state of employment
- BLS (CPR) at hire date, required, or within 90 days of hire
Desired Qualifications
- 2-3 years acute care hospital setting
- Care Management or Utilization Management experience
- Certification in Case Management
