Job Summary
Assess, plan, advocate, and coordinate patient care from admission to discharge while ensuring a safe transition post-hospitalization. Utilize performance metrics and best practices to oversee transition planning and length of stay. Maintain patient experience, safety, and quality of care as part of the interdisciplinary team. Requires a current RN license, 2-3 years acute care experience, and BLS certification. Part-time day shift in Lebanon.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current License in the state of employment
- BLS (CPR) at hire date
- BLS (CPR) within 90 days of hire
Desired Qualifications
- 2-3 years acute care hospital setting
- Care Management or Utilization Management experience
- Certification in Case Management
