Job Summary
Establish a well-rounded care practice by conducting patient assessments, developing care plans, implementing interventions, and monitoring ongoing evaluations to ensure medically and financially appropriate care. Facilitate interdisciplinary team care coordination and manage intensive medical support for participants recently discharged from hospitals or skilled nursing facilities. Collaborate closely with IDT social workers and physicians to gather necessary medical and social information for comprehensive safety and well-being decisions. Requires a Washington Registered Nurse license, one year of long-term care or geriatric nursing experience, and BLS certification. Monday-Friday day shift.
Required Qualifications
- Education sufficient for clinical licensure
- Washington Registered Nurse License
- National Provider BLS - American Heart Association
- 1 year Long term care or geriatric nursing
Desired Qualifications
- Bachelor's Degree in Nursing
- Medical surgical or rehabilitation medicine
