Job Summary
Conduct initial and periodic health assessments, monitor and coordinate supplies, equipment, and services, and lead care transitions including hospital-to-home discharge planning. Visit participant homes and participate in the on-call rotation while serving as a core member of the interdisciplinary team to shape care plans. Collaborate with staff, participants, and families to build strong, supportive relationships and provide education, advocacy, and leadership that empowers older adults to live independently. Apply sound clinical judgment to anticipate future care needs and establish proactive plans that evolve as conditions change, balancing participant choice, caregiver capacity, and regulatory requirements. This full-time, in-person role supports adults 55+ who qualify for nursing facility care to remain safe in their homes.
Required Qualifications
- Graduate of an accredited nursing program (Associate or Bachelor of Science in Nursing)
- Current, valid RN license in the State of Michigan
- Valid driver's license
- Strong critical thinking skills, with the ability to thrive in dynamic, evolving situations
