Job Summary
Conduct comprehensive assessments using DOH-approved tools to determine eligibility for medically necessary services and benefits. Coordinate enrollee services across plan benefits and Medicare, while developing and revising patient-centered plans of care. Educate enrollees on consents and documentation, and monitor physical, emotional, and environmental needs to prevent disease exacerbations. Implement physician orders for durable medical equipment and maintain all documentation within the Electronic Medical Record system. Adhere to state and federal regulations, including HIPAA, while managing conflict and multiple work demands. Requires a Registered Professional Nurse license in New York State, valid driver's license, and BLS certification.
Required Qualifications
- Computer literacy, including but not limited to data entry, retrieval, and report generation
- Excellent written, verbal and listening communication skills
- Strong organizational and time management skills
- Ability to work with patients/families of all ages and in a variety of settings, including inpatient facility and patients' homes presenting diverse physical conditions and social/cultural environments
- Ability to drive to and from a variety of settings in varying weather conditions
- Licensed and currently registered to practice as a Registered Professional Nurse or BSN in New York State
- Valid New York State driver's license and personal vehicle
- Current Basic Life Support (BLS) Certification
Desired Qualifications
- 1-2 years in community based care or health care area that cares for the community population
