Job Summary
Act as a liaison between patients, families, and healthcare personnel to ensure efficient services and minimize fragmentation. Develop comprehensive care plans addressing medical and psychosocial needs in collaboration with the SCO team to maintain cost-effectiveness. Control health center resources and home services through periodic reviews, maintain continuity of care between home, the health center, and hospitals, and provide nursing-centered office and home visits. Work with patients and families to adjudicate health care wishes, prioritizing independence and safety within the community. Requires RN licensure in Massachusetts, one year of community health experience, and bilingual skills preferred. Monday through Friday, 8:00 AM to 5:00 PM.
Required Qualifications
- Graduation from an accredited nursing program
- RN licensure in Massachusetts
- BLS certification
- At least one year of relevant nursing experience in a community health and/or ambulatory care setting
Desired Qualifications
- Bilingual skills
- Demonstrated ability to communicate effectively verbally and in writing
- Familiarity with Windows operating system
