Job Summary
Develop comprehensive care plans for members by addressing medical, social, psychological, physical, and spiritual needs in conjunction with primary care physicians, caregivers, and other team members. Implement and monitor these plans to ensure member needs are met effectively. Coordinate case management activities under the supervision of the Supervisor of Care Management. Requires an Associate's Degree in Nursing for experienced candidates or BSN for new graduates, along with a NYS RN license. Preferred qualifications include CCM certification and one year of RN experience in managed care, home care, acute care, or nursing homes. Salary ranges from $90,000 to $110,000 annually.
