RN Care Manager - Kenosha

Job Summary
RN Care/Case Manager collaborates with site management to standardize care delivery for Community Care members, conducts phone and in-person assessments using decision tools to identify health needs, and completes documentation while developing and maintaining member records. Requires an Associate degree in nursing, a Wisconsin RN license, and at least 2 years of experience in care or case management for frail/elderly populations. Full-time role in Kenosha County with 8:00 a.m.–4:30 p.m. schedule and a $8,000 sign-on bonus. Benefits include PTO, health/dental/vision, retirement, flexible benefits, and EEO employment. Travel is required.
Required Qualifications
- Education: Associate degree in nursing required.
- Experience: Minimum 2 years of personal, clinical leadership, and/or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
- Licensure: A current Wisconsin Registered Nurse license in good standing
