The role requires clinical expertise, strong communication with patients, families, and healthcare providers, and a focus on smooth transitions of care across settings. Preferred but not mandatory is Case Management certification.
Required Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing
- 1 year of acute care hospital setting required
- Current RN License in the state of employment required
- BLS (Basic Life Support) at hire date, or within 90 days (about 3 months) of hire
