Job Summary
RN Care Manager II-IPCM coordinates care from admission to discharge within an interdisciplinary team, assesses health needs, develops individualized care plans, and coordinates services to optimize outcomes. Communicates with patients, families, and healthcare providers; advocates for patient needs; promotes health education; and facilitates smooth transitions of care across settings. Requires clinical expertise, critical thinking, and compassion in alignment with Mercy mission and values.
Required Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing
- Current RN License in the state of employment
- BLS (Basic Life Support) at hire date, or within 90 days (about 3 months) of hire
- Preferred Certification: Certification in Case Management
- Preferred Experience: 2 years acute care hospital setting preferred. Care Management or Utilization Management experience preferred.
- Required Experience: 1 year of acute care hospital setting required.
