Job Summary
RN Care Manager II - IPCM assesses, plans, advocates, and coordinates care from admission to discharge, ensuring a safe transition post-hospitalization. Develops individualized care plans, coordinates services to optimize outcomes, and communicates with patients, families, and healthcare providers. Plays a key role in advocating for patient needs, promoting health education, and facilitating smooth transitions of care across settings. Requires an accredited nursing degree, active RN license in the employment state, and BLS within 90 days of hire; 1 year of acute care experience is required. Mercy mission and values guide duties.
Required Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing
- 1 year of acute care hospital setting required
- Current RN License in the state of employment required
- BLS (Basic Life Support) at hire date, or within 90 days of hire
