Job Summary
RN Care Manager II-IPCM coordinates care from admission to discharge, assesses patients' health needs, develops individualized care plans, coordinates services, advocates for patients, and facilitates smooth transitions of care across healthcare settings. Communicates effectively with patients, families, and healthcare providers to promote health education and optimal outcomes. Requires BSN, RN license, and BLS; 1 year of acute care experience; preferred Certification in Case Management.
Required Qualifications
- Graduate of an accredited School of Nursing with Bachelor’s Degree of Science in Nursing (BSN)
- Current RN License in the state of employment
- 1 year of acute care hospital setting required
- BLS (Basic Life Support) certification at hire date or within 90 days of hire
