Job Summary
RN Care Manager II-IPCM coordinates post-hospitalization care, assesses patient needs, develops individualized care plans, advocates for patients, and coordinates services to optimize health outcomes. Works with an interdisciplinary team to ensure safe transitions of care from admission to discharge, communicating with patients, families, and other healthcare providers. Requires a BSN, current RN license, and 1 year of acute care experience; BLS certification at hire date or within 90 days; preferred Case Management certification and additional acute care/ utilization management experience. Required Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing
- 1 year of acute care hospital setting
- Current RN License in the state of employment
- BLS (Basic Life Support) at hire date, or within 90 days
