RN Care Manager II-IPCM - Days - PRN

Assess, plan, advocate, and coordinate care from admission to discharge ensuring a safe transition post hospitalization. Perform duties consistent with the Mercy mission, values, and service standards. Utilize the Care Management model to provide effective transition planning and length of stay oversight while maintaining patient experience, safety, and quality of care through performance metrics and best practices. Work within an interdisciplinary team to support the healing ministry through compassionate care and exceptional service.
