RN Care Manager II-IPCM - Days - PRN

Assess, plan, advocate, and coordinate care from admission to discharge while ensuring a safe transition post hospitalization. Perform duties consistent with the Mercy mission and service standards within the interdisciplinary team. Utilize performance metrics and best practices to oversee length of stay and maintain patient experience, safety, and quality of care.
