RN Care Manager

RN Care Manager provides hospice nursing care to patients and families in Licking County as part of an interdisciplinary team. Responsibilities include planning, coordinating, and delivering patient care using the nursing process (assessment, planning, implementation, intervention, evaluation) and collaborating with patients, families, physicians, and other healthcare professionals to ensure compassionate, patient-centered care. Full-time role in Newark, OH with schedule Mon–Fri, 8:00 a.m.–4:30 p.m., rotating holidays and every 8th weekend. Travel required; driving position with driving license and safe driving record. Required: RN license in Ohio, ability to pass background check and 10-panel drug screening; CPR certification; basic computer skills; willingness to travel; state automobile insurance.
