Job Summary
Conduct clinical assessments and medication reconciliation to determine appropriate care management needs. Develop, implement, and evaluate Individualized Care Plans (ICP) while acting as a resource for clients, providers, and facilities. Serve as a Member advocate and liaison to educate caregivers on meeting ICP goals and assist with referrals for carve-out programs, access, authorizations, and community resources. Organize Interdisciplinary Care Teams to ensure facilitation and documentation. Review contributions from care team members and manage care management processes to ensure successful Member outcomes. Requires a valid unrestricted RN license and California driver's license with minimum two years of clinical experience.
Required Qualifications
- Valid, unrestricted RN license issued by the State of California
- Possession of a valid State of California driver's license
- Minimum 2 years clinical experience in an acute care facility, skilled nursing facility, home health or clinic setting
Desired Qualifications
- Two (2) or more years of care management experience in a health care delivery setting
- Experience in a Managed Care (HMO, IPA) or in acute facility (i.e. hospital) care management
