RN Care Manager - Full Time

Job Summary
Manage a small private-pay caseload of older adults in homes and senior communities as the primary RN point of contact. Perform in-depth medical, psychosocial, and environmental assessments to develop individualized care plans focusing on safety, chronic disease management, and quality of life. Coordinate services including home health, therapy, and durable medical equipment while collaborating with physicians, facilities, and community partners to ensure smooth transitions of care. Provide ongoing monitoring through regular visits, crisis intervention for urgent situations, and emotional support to clients and families. Thoroughly document all assessments and interventions to support billing and clear communication. Partner with the business development team to explain RN-led care services to prospective clients and families.
Required Qualifications
- Licensed Registered Nurse (RN)
- Passion for working with older adults and their families
- Comfortable using clinical judgment in a community/field‐based setting rather than in acute care
- Strong communication skills to translate medical information into clear, actionable plans for families
- Organized and self‐directed
- Ability to manage a mobile schedule, documentation, and follow‐up
- Relationship‐builder skills for collaborating with physicians, facilities, and community partners
